Healing Sky Provider Tax Forms Guide
The Tax Forms section allows providers to securely submit and manage their W-9 information for tax reporting purposes. This information is required to receive payouts and ensures compliance with U.S. tax regulations.
Keeping this section accurate and up to date is essential for uninterrupted payments.
Where to Find Tax Forms
From the Provider Dashboard:
My Account → Tax Forms
This page allows you to:
- Submit your W-9 information
- Upload identity verification documents
- Complete required tax compliance steps
W-9 Form Information
At the top of the page, you will see two tabs:
- Individual

Business
Select the option that applies to your tax filing status.
Individual (U.S. Citizens or Residents) – W-9 Form Info
If you are filing as an individual, you will complete the following fields:
Full Legal Name
Enter your name exactly as it appears on your tax return.
Why this matters:
This ensures your earnings are properly reported to the IRS.
SSN / ITIN
Enter your Social Security Number or Individual Taxpayer Identification Number.
Why this matters:
This is required for IRS reporting and 1099 generation.
Address
Enter your current mailing address.
Why this matters:
This ensures accurate tax documentation and official communication.
Email (Optional)
Provide an email address for tax-related correspondence.
Phone (Optional)
Provide a contact number if needed for verification.
Certification Checkbox
You must check the box confirming:
“I certify that the above information is correct.”

This acts as your legal acknowledgment that the information provided is accurate.
Full Name (Signature Confirmation Field)
Re-enter your full name to confirm identity.
Today’s Date
Select or confirm the current date.
Signature
Sign electronically in the signature box.
Why this matters:
This finalizes your W-9 submission and serves as your digital authorization.
Personal Identity Verification
Below the W-9 section is Personal Identity Verification. This helps confirm your identity for compliance and security purposes.
You will see three verification areas:
Government-Issued ID
Upload a valid government-issued ID (PDF format supported).
Examples:
- Driver’s license
- Passport

At the bottom of the page, click:
